OxLife
oxygen concentrators, which are manufactured
and sold by OxLife, are covered as following:
OxLife extends the warranty to the original
purchaser only unless a writ of warranty transfer
is issued (See Warranty Transfer).
NORTH
AMERICA OXLIFE WARRANTY
This
warranty guarantees OxLife concentrators to
be free of defect in workmanship and material
from the date of shipment from OxLife to the
original purchaser. This warranty extends
for three years for parts and one year in labor
unless extended warranty is purchased (See Extended
Warranty). This OxLife Warranty DOES NOT
include the following:
Ø
Any concentrator sold by OxLife that is not
considered NEW at the time of purchase. For
all concentrators that are not considered NEW,
see USED CONCENTRATOR WARRANTY.
Ø
Any OxLife concentrator purchased overseas.
(See Foreign Warranty)
Ø
Any maintenance parts (filters, valve diaphragms,
and compressor wear parts)
Ø
Any damage done to the OxLife Concentrator due
to abuse, mis-handling, or not operating in
accordance with OxLife operating and service
instructions (See Service Manual). Doing this
can void the warranty.
USED
OXLIFE CONCENTRATOR
OxLife
will guarantee used OxLife Concentrators (Excel,
L-3, L-6, or Freedom 5) for a period of one year to be
free of defect in workmanship and material from
the date of shipment from OxLife to the original
purchaser. Within that one year the OxLife concentrator
should need service, the Used OxLife Warranty
will cover any parts and labor needed to service
the concentrator. The purchaser will be
responsible for ground freight to send to OxLife.
OxLife will pay for return freight.
OVERSEAS
OXLIFE WARRANTY (See Attachment)
If
the need does arise to have your OxLife Concentrator
serviced, see “How To Get Service”
in your service Manual.
Ø
Call OxLife at 1-800-780-2616 to obtain a Return
Authorization Number or OxLife will direct the
owner of the OxLife concentrator to the nearest
Authorized Service Dealer. OxLife is not
responsible for labor incurred if a non-authorized
service dealer performs the warranty work on
the OxLife concentrator. If unit is shipped
to OxLife, please do the following:
Ø
Place the concentrator in the original box and
packing to ship back. If box is not available,
call OxLife to obtain a new box.

Write Return Authorization Number on the outside
of the box and ship to:

Ø
Owner of the OxLife Concentrator will pay for
shipping to OxLife. OxLife will be responsible
for freight back to the owner (Continental United
States Only). For Alaska and Hawaii owner pays
full freight.
To
issue a warranty claim, the owner of the OxLife
concentrator must notify OxLife at
1-800-780-2616. The claim will be evaluated,
and if determined that service must be rendered,
further instructions will be issued (via a Return
Authorization Number or the name of a Warranty
Repair Facility). Upon receiving the OxLife
concentrator, OxLife technicians will evaluate
the concentrator. If found that the concentrator
has been damaged due to abuse by the user, the
warranty will be void and the owner will be
responsible for parts, labor and return shipping.
Optional
Warranty
The
OxLife Optional Warranty covers labor for the
full three years. In the event that your
concentrator needs repair, the OxLife Optional
Warranty will cover the complete cost of the
concentrator, minus the cost of shipping the
concentrator to OxLife or one of OxLife’s
Authorized Warranty Repair Centers.
A
free loaner concentrator, direct from OxLife
will be provided (customer must pay all freight).
**This
warranty applies only to Continental USA sales
on return freight.
NOTE:
The optional warranty must be selected at the
time of purchase.
THERE
ARE NO OTHER WARRANTIES, IMPLIED OR DIRECT,
THAT ARE IN EFFECT FOR THE OXLIFE CONCENTRATORS.
ALL OTHER WARRANTIES ARE EXCLUDED. THIS
IS THE EXCLUSIVE REMEDY AND LIABILITY FOR CONSEQUENTIAL
AND INCIDENTAL DAMAGES UNDER ANY AND ALL WARRANTIES
ARE EXCLUDED TO THE EXTENT EXCLUSION IS PERMITTED
BY LAW. SOME STATES DO NOT ALLOW LIMITATIONS
ON HOW LONG AN IMPLIED WARRANTY LASTS, OR THE
LIMITATION OR EXCLUSION OF CONSEQUENTIAL OR
INCIDENTAL DAMAGE. THE ABOVE LIMITATIONS
OR EXCLUSION MAY NOT APPLY TO YOU.
This
warranty gives you specific legal rights, and
you may also have other rights which vary from
state to state.
Please
fill in the following information and return
to OxLife. The purchaser has 15 days after receiving
their OxLife concentrator to return the OxLife
Warranty to OxLife or OxLife has the right to
